[Music]
very often we find parents coming and
telling me
that you know doctor my child was
wheezing and when we do evaluate we
recognize
that the sound was coming primarily from
the nasal system which is the nose of
the child
wheezing on the contrary is a sound that
comes
when the vein pipes which are present
inside our chest
in the lungs start to narrow down and
there is a whoosh like sound that is
produced
from the turbulence in the air movement
within these pipes
so it is important that we differentiate
whether the noise in a child is coming
from the nose
which is not so harmful and not so
buddhism
and whether or not it is coming from the
narrowing
or the turbulence of air movement in a
narrowed windpipe
within the chest of the child and there
are various reasons
why these windpipes may get narrowed so
if we were to imagine
a pipe a pipe can get narrowed because
something inside this
gets stuck and there is a clogging of
the pipe
which can happen from a lot of our own
breathing secretions
and sometimes in children it may also
happen when they accidentally
inhale a foreign particle a foreign
object
such as a tiny piece of a toy and
sometimes the wheezing can happen
because the walls of these windpipes
seem to swell up
and sometimes the wheezing can be
contributed by reasons that the windpipe
might get
pressurized from outside all of these
causes will compromise the airflow
and the child will struggle to breathe
thereby the
causing all of these noises and
therefore identification of a wheeze is
very very critical
because any intervention to open up the
airway
or the windpipe is instrumental in
saving
life of the child
[Music]
as we put our ear on to the child's
chest
very often there are these harsh horse
loud sounds which resonate in a child's
breathing movement
and they are often snoring like sounds
coming from the nostril
wheezing on the contrary is a whistling
kind of a sound
which is very distinct and it is not so
loud
not like a snow not very harsh and
sounds something like this
[Music]
it is important for parents to identify
the effort
or put by the child while trying to
breathe with any of these sounds
very often when the nose is blocked a
child would tend to breathe
trying to keep its mouth open so that it
can bypass the blockage and try to take
in air inside
with wheezing on the contrary there is
little that a child can do
because the windpipe is the only passage
which
can which works between the nose to
provide us
air throughout to the bloodstream so the
child is going to put
all its might and all the effort into
breathing
many times when we hear these whoosh or
whistling sounds
we will also find children coughing very
frequently
and not able to converse in proper
sentences they will be
in effect what we call breathless so
they will often run out of breath
for those children or infants and
toddlers who are not talking and not
moving around
if we observe them closely as we are
trying to breathe
we often find them taking a lot of deep
breaths or you could also call them deep
efforts
and there's a lot of effort which
results in inward movements of the
muscles around the neck
and sometimes there is inward movements
of a lot of muscles between the rib
spaces
so what is also called in loken paladins
as parsley chana
yeah sas fullna is a very important
determinant of
breathing difficulty or distress in a
child especially when it accompanies
these sounds
